Coach Evaluation Form

In order to improve as a youth soccer organization we would like some feedback from parents and players to assess the effectiveness and the abilities of our coaching staff over the past season. We believe that it is important for you to be honest and fair in your assessment. The Club Dosveedanya executive board and the Director of Coaching need to know what areas may need to be improved, as well as recognize the positive qualities of each of our coaches.

Note: Coach evaluation closes upon completion of each team's State Cup.

Use the following scale to answer each of the questions below (Note: Some questions may not fit the wording of this scale, but please use it as a guideline): 5=Excellent 4=Good 3=Average 2=Poor 1=Very Poor
	Were practices well organized?
	Does this coach effectively communicate and demonstrate the concepts he/she is trying to teach?
	Did this coach display and encourage respect for game officials?
	Did this coach display encouraging and positive conduct on the field?
	Did this coach show a positive attitude with all players?
	Did this coach motivate your daughter in a positive manner?
	Did this coach communicate “age appropriately” with the players?
	Did this coach communicate well with the parents?
	Do you feel your daughter showed patterns of improvement because of the coaching she recieved from your current coach?
	Did you feel satisfied with your coach’s attendance during practices and games?
